Job Description
- Evaluate clients’ physical, mental, and emotional conditions to design tailored rehab strategies.
- Work closely with healthcare teams to implement and monitor rehabilitation plans.
- Guide patients through therapeutic exercises, daily routines, and lifestyle adjustments.
- Track client progress and adjust plans based on changing needs and feedback.
- Educate families and caregivers on proper support techniques and post-rehab care.
- Connect clients with resources like vocational training, community support, or assistive devices.
