A Social Worker Assistant supports licensed social workers in providing care and resources to patients facing social, emotional, or financial challenges. Often working in hospitals, clinics, or care homes, they assist with case management, referrals, and follow-ups. This role is key to improving patients’ wellbeing and linking them with essential services. They also maintain records and ensure timely intervention. It is both emotionally fulfilling and logistically demanding.

Job Description
- Assist social workers with documentation and case monitoring.
- Help patients access financial aid, housing, or mental health services.
- Arrange transportation or appointments as needed.
- Support family meetings and care planning discussions.
- Maintain records of interactions and service referrals.
- Monitor clients’ progress and report to the lead social worker.
- Offer compassionate listening and basic counseling support.
