Job Description
- Assess clients’ needs, limitations, strengths, and career goals through interviews and tests.
- Develop personalized rehabilitation plans focusing on education, employment, and emotional wellbeing.
- Provide counseling to help clients cope with disabilities, trauma, or lifestyle changes.
- Coordinate with medical teams, employers, and social workers to ensure holistic support.
- Help clients find training programs, job placements, or community resources.
- Maintain detailed case notes, progress reports, and ensure compliance with confidentiality policies.
