OT Scrub Nurse

An OT Scrub Nurse is responsible for preparing and maintaining the sterile field during surgeries, ensuring that all instruments, sutures, and consumables are correctly arranged and sterile. They scrub in with the surgical team, assist the surgeon during operative steps, anticipate instrument needs, and ensure accurate surgical counts before, during, and after procedures. Their role also includes preventing contamination, managing specimens, handling surgical equipment, and coordinating closely with circulating nurses and anesthesia staff to maintain safe and efficient intraoperative care.

Patient safety is maintained through strict adherence to aseptic principles, instrument sterility checks, and WHO Surgical Safety Checklist compliance. The scrub nurse verifies patient identity, surgical site, and procedure details during the “Time-Out.” Sterile barriers are maintained throughout surgery, and appropriate positioning, padding, and preparation solutions are ensured to minimize complications. Monitoring instrument integrity, preventing retained surgical items, and ensuring correct instrument counts further enhance intraoperative safety.

Instrument and swab counts are conducted at three stages: before skin incision, during closure of cavities, and at the end of surgery. The scrub nurse collaborates with the circulating nurse to complete counts, document them, and immediately report discrepancies. Use of count boards, radiopaque materials, and standardized checklists ensures accuracy. In case of mismatches, the scrub nurse halts closure steps and follows the hospital protocol for locating missing items.

In complex surgeries, the scrub nurse prepares advanced instrument sets, energy devices, staplers, and endoscopic equipment. They anticipate surgeon needs based on surgical phases and ensure timely instrument transfer to maintain operative flow. For laparoscopic procedures, they prepare trocars, camera systems, insufflation settings, and endoscopic instruments, ensuring all equipment functions correctly. They also help maintain visualization, manage specimen retrieval, and troubleshoot instrument issues.

Prevention includes strict adherence to sterile technique, maintaining hand hygiene, using sterile gowns and gloves, controlling traffic flow in the OT, and ensuring proper sterilization of instruments. The scrub nurse avoids touching non-sterile surfaces, ensures proper draping, and maintains sterile barriers throughout the surgery. Immediate correction of breaks in sterility and communication with the surgical team help minimize infection risk.

Candidates must complete GNM, BSc Nursing, or PB BSc Nursing, along with valid registration under the State Nursing Council. Strong clinical understanding of surgical anatomy, aseptic practices, instrument names and usage, surgical safety protocols, patient positioning, and intraoperative clinical workflows is essential. Many hospitals prefer nurses with prior exposure to operating rooms, surgical wards, or emergency care due to the high-acuity nature of OT environments.

While it is not strictly mandatory, perioperative nursing training, scrub nurse training, or hospital-based OT certification significantly enhances competency. Specialized courses such as AORN Perioperative Modules, Laparoscopic OT Training, Biomedical Equipment Training, and Endoscopic Surgery Assistance strengthen clinical skills. These programs improve understanding of sterile techniques, advanced instrument handling, diathermy safety, and intraoperative decision-making.

The salary of an OT Scrub Nurse in India varies according to experience, hospital category, and surgical specialty exposure. Entry-level scrub nurses typically earn between ₹2,40,000 and ₹3,60,000 per year, while those with 2–4 years of experience earn between ₹3,60,000 and ₹5,40,000 annually. Senior scrub nurses working in super-specialty areas like cardiac OT, neuro OT, transplant OT, or advanced laparoscopic units may earn ₹5,40,000 to ₹7,20,000 or more. Hospitals with high surgical workloads or 24×7 emergency services generally offer higher pay.
