Job Description
- Monitor patients’ anticoagulation therapy through lab results (e.g., INR) and clinical assessments, adjusting dosages accordingly.
- Counsel patients on medication adherence, dietary considerations, and lifestyle factors impacting anticoagulation therapy.
- Collaborate with physicians and healthcare teams to develop and adjust individualized anticoagulation management plans.
- Identify and manage potential drug interactions, side effects, and patient-specific risks related to anticoagulation.
- Maintain detailed documentation and contribute to quality improvement initiatives within the anticoagulation clinic.
