Job Description
- Coordinate individualized care plans for oncology patients in collaboration with the healthcare team.
- Monitor patient progress, manage symptoms, and ensure adherence to treatment protocols.
- Educate patients and their families about diagnosis, treatment options, side effects, and follow-up care.
- Provide emotional support and counseling, addressing psychosocial needs throughout the care continuum.
- Facilitate communication between oncologists, surgeons, radiologists, and support services.
- Assist in managing transitions of care, including discharge planning and home care coordination.
- Maintain detailed and accurate medical records in compliance with clinical and ethical standards.
