Job Description
- Assess cardiovascular risk factors such as hypertension, diabetes, obesity, smoking, sedentary lifestyle, and family history.
- Develop personalized risk-reduction plans, including diet, exercise, smoking cessation, and medication strategies.
- Monitor patients with high-risk profiles, providing ongoing support, adjustments in therapy, and regular follow-ups.
- Use diagnostic tools (e.g., lipid panels, ECGs, blood pressure monitoring, coronary calcium scoring) to evaluate heart disease risk.
- Educate patients and communities on cardiovascular health, prevention strategies, and early signs of heart disease.
- Collaborate with cardiologists, endocrinologists, dietitians, and primary care providers to ensure coordinated, multidisciplinary care.
- Implement and oversee wellness programs, especially in corporate, clinical, or public health settings, to reduce population-level heart disease burden.
