Job Description
  1. Risk Assessment and Evaluation
  2. Conduct cardiovascular risk assessments using tools such as cholesterol panels, blood pressure checks, and lifestyle surveys. Identify individuals at risk for cardiovascular diseases based on family history, lifestyle choices, and existing health conditions (e.g., diabetes, hypertension).
  3. Education on Cardiovascular Risk Factors
  4. Educate individuals and groups about the major risk factors for cardiovascular diseases, including high blood pressure, high cholesterol, smoking, physical inactivity, poor diet, and excessive alcohol consumption. Provide personalized information on how these factors contribute to heart disease and stroke.
  5. Developing Risk-Reduction Plans
  6. Help at-risk individuals develop personalized action plans to reduce their cardiovascular risk. This may include recommendations for improving diet, increasing physical activity, quitting smoking, managing stress, and controlling blood pressure and blood sugar levels.
  7. Lifestyle Modification Coaching
  8. Provide one-on-one or group counseling on healthy lifestyle changes such as adopting a heart-healthy diet (low in saturated fats, high in fruits, vegetables, and whole grains), engaging in regular exercise, and practicing stress management techniques. Offer practical tips for incorporating these changes into daily life.
  9. Promotion of Preventive Health Screenings
  10. Encourage individuals to participate in routine health screenings (e.g., blood pressure, cholesterol, glucose) and monitor key biomarkers associated with cardiovascular health. Guide individuals on how to interpret these results and discuss appropriate next steps if any abnormal findings occur.
  11. Collaboration with Healthcare Teams
  12. Collaborate with physicians, dietitians, exercise physiologists, and other healthcare providers to offer a multidisciplinary approach to cardiovascular health. Ensure that educational materials and risk-reduction strategies are consistent with the advice and recommendations of the healthcare team.
  13. Community Outreach and Group Programs
  14. Organize and facilitate workshops, seminars, and health promotion campaigns aimed at increasing awareness about cardiovascular health. Lead group activities that promote heart-healthy behaviors, such as fitness challenges, cooking classes, and smoking cessation programs.
