- Supporting patients in adapting to lifestyle changes for long-term health management.
- Educating patients and caregivers about disease management, assistive devices, and home safety.
- Collaborating with multidisciplinary teams, including cardiologists, pulmonologists, and physical therapists.
- Monitoring progress and adjusting care plans based on individual recovery.
- Promoting mental well-being by addressing stress and anxiety linked to chronic conditions.
