Job Description
- Assess the health, social, emotional, and financial needs of elderly clients to create personalized care plans.
- Coordinate medical appointments, in-home care, transportation, and other essential services for seniors.
- Serve as the primary point of contact between healthcare providers, family members, and community resources.
- Monitor clients’ well-being and adjust care plans as their needs and circumstances evolve.
- Provide guidance, counselling, and support to families navigating the complexities of elder care.
