Job Description
  1. Medication Therapy Management – Assess and manage patients' medication regimens to ensure that drugs prescribed are appropriate, effective, and safe. This involves reviewing the patient’s medical history, current medications, and potential drug interactions.
  2. Clinical Consultation and Advice – Provide expert advice to healthcare providers (doctors, nurses, etc.) on the selection and dosing of medications, therapeutic drug monitoring, and how best to manage complex pharmacological treatments for various conditions, including chronic diseases.
  3. Patient-Specific Therapeutic Recommendations – Develop individualized pharmacotherapy plans based on patient-specific factors such as age, genetics, comorbidities, and organ function. Recommend adjustments to drug regimens to optimize treatment outcomes.
  4. Monitoring Drug Efficacy and Safety – Monitor patients’ responses to therapy through follow-up evaluations, lab tests, and clinical assessments. Adjust medication plans as necessary to ensure optimal drug effectiveness and to mitigate adverse effects.
  5. Education and Counseling – Educate patients on the proper use of their medications, including potential side effects, drug interactions, and the importance of adherence to prescribed therapies. Provide counseling on lifestyle changes or other health interventions to complement pharmacotherapy.
  6. Collaborative Care in Multidisciplinary Teams – Collaborate with other healthcare professionals, such as physicians, nurses, and dietitians, to provide comprehensive care. Participate in case discussions and contribute insights into pharmacological treatments for various medical conditions.
  7. Research and Evidence-Based Practice – Stay up-to-date with the latest pharmacological research, drug therapies, and treatment guidelines. Engage in evidence-based practice to continuously refine and improve therapeutic strategies and patient care.
