Job Description
- Provide emotional and psychological support to individuals coping with the stress, anxiety, or depression associated with diabetes.
- Help patients develop healthy habits, such as regular physical activity, balanced nutrition, and medication adherence.
- Counsel on behavior change strategies, goal setting, and overcoming barriers to effective diabetes self-care.
- Educate patients and families about diabetes management basics and promote empowerment in daily decision-making.
- Collaborate with healthcare teams, including physicians, nurse educators, and dietitians, to provide holistic diabetes care.
- Support patients in crisis situations, such as burnout, fear of hypoglycemia, or major lifestyle adjustments post-diagnosis.
- Facilitate support groups or community workshops to reduce isolation and encourage peer learning.
